The first visit to The Brown Spot will be your dog's evaluation. An acceptable evaluation will allow your dog to join the spot for all services. Please complete the forms below (Questionnaire, Liability Waiver & Service Agreement) and text or email up-to-date vaccination records to info@thebrownspotllc.com. Once the completed documents are received we will contact you to schedule your evaluation !!
All dogs are required to be human and dog friendly. We understand that all dogs may not get along with all other dogs.
We require proof of the below vaccines for all dogs;
RABIES, DISTEMPER, PARVO & BORDETELLA
